You have to be more precise when you describe a problem. Do you see high CPU usage for the inetinfo.exe while trying to create the project? Review this article and see if you are not running into any known issues.

Are you running front page extensions? Visual Studio 2003 likes to use front page extensions to creat web applications in IIS. Make sure front page are a) Installed b) Up-to-date.
As for your database problem what type of authentication are you using Windows or Mixed Mode (SQL and Windows)? If your using windows authentication make sure the user account you are logged in with has a password specified and that account has access to

You can install the remaining software in any order once you have the OS installed. Installing IIS before VS.NET 2003 saves you the trouble of registering ASP.NET in IIS by running the aspnet_regiis command. SQL Server is not dependent on IIS or VS.NET2003
so it can be installed anytime.
