I am considering user Azure as the Authorization Server so that I can leverage Azure AD for Accounts and user management.
If I do not chose to go that route I am thinking to use ASP.NET Identity / SQL Server and
Identity Reboot to manage users.
For older sites (when leveraging SQL Server) I used the Membership Provider in .NET and the SQL Server Database. Users and Roles where managed using Web Site Admin Tool (screen shot below).
Question: If I do not go with Azure for the Auth Server, is ASP.NET Identity (the replacement for the former Membership Provider) a good route to go if I want to manage users using SQL Server?

Yes, ASP.NET Identity is for the management of users, roles and memberships no matter what you are using at backend i.e. databse or Azure or whatever. Please have a look at following links.
If you want to go with SQL Server that will also work.

You can develop a custom membership solution based on ASP.NET Identity and utilizing Database First or Code First development approach.
