What type of application is the login form for? Web Forms? MVC? What exactly is the problem you are facing?

Why don't you just use the Login Control?

I want to implement design and patterns in my application. I thought that it is easy to analyze with login form.

Login Forms don't really lend themselves to design patterns.
What you should do is learn what design patterns there are and what problems they solve. Then you will know which one to use and when you need to use it. The problems should present themselves. If you go around hunting for problems in everything you do just
so you can fit a design pattern in, you are doing it wrong.

refer http://www.dofactory.com/Default.aspx to read about the design patterns. Once you are familier with design patters, you yourself can decide about the best suitable pattern for your requirement.
