First thing while use crystal report is path. always give in web.config of your physical file path.
Second while export report, it needed your local temp path. Good to use again in web.config declaration. Also this temp folder needed the full permission to read/write.
Now your issue of "Missing parameter",
1. check that you don't save as "Save with data" (Menu:- file -> save with data) .if tick , then remove.
2. try to check sqlserver connectivity with deploy report. the error show your parameter is not passed , may be wrong sqlserver db or wrong sp call. To verify just open report , f5, if parameter then give value and check result. It is fine.
3. In your code, dynamically pass connection string, again the same thing must configure in web.config.(http://stackoverflow.com/questions/2784836/using-a-connection-string-in-web-config-for-crystal-report
