May be you are working as a freelancer, but would suggest that customer handle the database since he is the owner of the application. Here are some questions to you
Are you on a static IP ? Does the server that hosts enough rig to go public ? Does the db server have domain name ?
Yes I would suggest you use a WCF service and have proper security in place. The service will be consumed by the app in cloud platform. this is overrall thought, but you might face more problems on your way to implement this

Yes. You can put your data access logic inside wcf service and call that service reference to your application.

I am deverloping a cloud service which allow your WCF in local network be accessed from internet , check
http://homehttp.com for the details.
I also made a new open source CDN project , will publish to codeplex soon.
If you want to use custom way , you can setup the router , make port mapping , and find a dynamic DNS solution if your local public IP often change .
So that you can access your local website like service.Url="http://yournetwork.somedynamicdnsprovider.com/"+servicefile;
