All of these badges/titles are given at the descretion of Microsoft, the Community as a whole or through Certification Testing.
MVP (Most Valuable Professional) This is the highest award given by Microsoft to those it considers "the best and brightest from technology communities around the world" who "actively share their technical expertise with the community and with Microsoft". An MVP
is awarded for contributions over the previous year. (Anyone can nominate themselves or be nominated by their peers, but Microsoft will chose those they deem worthy.)
MCP (Microsoft Certified Professional) This title involved being certified through a series of tests in specific areas to test the person's skill and ability within a certain realm of skills, such as Development, Technical Expertise, etc. (Anyone is eligible to sign up and take
these tests.)
Moderator This title is gained by being nominated by the Community as an active and responsible member who has access to controlling content and dealing with issues within the Community.
ASP Insider These users are selected to provide feedback on the .NET series of Products. They are typically chosen based on their contributions to the Community.

I didn't mention any certification required to become an MVP.
Anyone is eligible as long as they were nominated and Microsoft feels that their accomplishments merit the award.
