I have project A loaded into Visual Studio 2003. I have project B loaded into another instance of Visual Studio 2003. Project A uses DLLs that are compiled in project B. If I start debugging project A, how can I get the debugger to switch back and forth
between both instances of Visual Studio 2003 in order to debug both project A and project B at the same time?

I don't know if you can do this while running two separate instances of VS, because they will have their own executions.
You could begin a new 'Solution' and include your two projects under the one solution?
