I am using this HTTP Handler.
Is that still an option in IIS 6/7? Do you think Hosting companies ever use it?

That setting exists in IIS6 so just make sure that your host has it disabled for you. Most hosts will support changing it if you ask. Both IIS5 and IIS6 have this check disabled by default for the .aspx extension, so you shouldn't have a problem with it
unless it was purposefully added.
As far as IIS7, the file mappings follow a whole different concept. As far as I know, you can't set it to fail based on if the file exists directly from IIS. It will hand off the request to ASP.NET properly whether the file exists or not.
