Look in following places to see what is going on.
- Check your web.config file, does it disabled debug.
- Check your IIS virtual directory, is it an application or not, and properly set to use appropriate Framework version.
- You have already check other item, but reconfirm.

Yes agreed, strange it seems. I have experienced such problems on some occasions, but not much. Then I use one of either, Run the application in debug mode, or attach to process for debugging. One of them always worked for me in all those occasions.

You attach to the Web server process (inetinfo.exe) or to the ASP.NET worker process if running in isolation.
