  FWIW, I do find I usually have to rebuild solution twice from a clean source install, but thats the only real anomoloy I have observed.

  It sounds like the windows user or group to which the user belongs does not have the appropriate ACL's on the image. I would check the permissions on said image. Also make sure that ASP.NET account has the appropriate permissions. The name of this account varies depending on your platform pre 2003 it will be ASPNET, 2003 will be Network Services
