What you're looking for is not available in the core. I've had great success with Tams mod. The download will not show up until you register on his site.

There is a Windows Authentication add in located at www.tttcompany.com . I haven't tried the new version but the one for DNN2 works great.

SQL Server comes with a copy of Enterprise Manager on the disk. It's pretty straightforward once you have the interface installed.

Re: How to manually install the database (DNN212)
Yes, you can manually run the scripts. I believe they are located in the dotnetnuke\providers\dataproviders directory. You do not have to rename the database. You can select any name you'd like for the database and declare it in the web.config file.
