I ran into and purchased this document application called Document! X from Innovasys. I just wanted to pass on that the app does a very good job of documenting your apps and DBs. It integrates with VS, but is also a standalone app (kind of the way SourceSafe works). I've only had 4.0 for a few days but I feel pretty confident that it is a cool documentation tool.
