If you are looking for something specialized, perhaps you may want to try LegalZoom.com or contact a local attorney near you. The third and cheapest option is of course a google search but I am not sure if you will find specific for a programmer under contract.
Those parts you would need to write out. Which is why you might want an attorney to write this thing out for you. True, there is a cost associated but at least you would be assured all your bases are covered.
